About The Position

The Outreach and Intake Case Manager will determine Outreach Services program eligibility and will provide an assessment of social, psychological, and environmental factors in order to determine the needs of the client for linkage to needed services/resources. Working with the client, client's significant other, family, faith based community and other community providers, the case manager will provide/facilitate access to resources to meet the client's needs. Client follow-up is an integral function of job.

Requirements

  • A minimum of 2 years of completed college credit is required.
  • Minimum of 2 years' experience, preferably in case management and practice in the social service field with knowledge, sensitivity, and experience focused on homelessness, domestic violence, trauma, substance abuse, human trafficking, mental illness and/or other challenges.
  • Knowledgeable in Word, Outlook, and other computer programs needed to fulfill job requirements, as well as an ability to document in an electronic medical record system.
  • Ability to work effectively in a fast-paced and dynamic environment with flexibility in scheduling while professionally managing the physical and emotional traumas of clients.

Nice To Haves

  • A Bachelor's Degree in social work, psychology or related area is preferred.

Responsibilities

  • Provides intake screening and assessment at Swope Central and homeless sites including non-traditional sites such as streets and homeless camps. The Outreach and Intake Case Manager will work with the Outreach Supervisor and/or Team to schedule regular outreaches in the community for the purpose of identifying eligible clients and enrolling them in the program.
  • Determines homeless eligibility and mainstream resource eligibility.
  • Assists clients in determining needs and provides information/education on services available at Swope and in the community to meet client's needs.
  • Provides intensive case management for Outreach clients at Swope Central, homeless sites, client's home or wherever the resource or service emanates. Connect the client to resources, supports, and services and provide appropriate referrals; i.e.: Medicaid, entitlements, Shelter Plus Care, etc.
  • Provides or refers clients for follow up to ensure needed services are received, for example, by reminding clients about appointments, warmly handing the client off to the Addictions Counselor/QM HP, helping collect necessary documentation from shelters; and promoting client understanding of instructions from medical/behavioral health providers and other community service providers.
  • Coordinates all aspects of needed services such as housing placement, substance abuse treatment, medication management, medical/behavioral health appointments, benefits applications, skill training, employment services, and clean and sober socialization.
  • Develops, reviews and updates a treatment plan or course of action in collaboration with the client, utilizing a strength-based model for on-going services.
  • Documents all client encounters/case management activities in the client record in a timely manner, and completes other paper-work and documentation as required by grantors, Swope Health Services, Supervisor, and other Swope Health Services Departments and Collaborators.
  • Facilitates group case management at Swope Central, Satellite Clinics, and Outreach as agreed by Supervisor and in collaboration with the Outreach Services Team. Examples include groups to address relevant issues such as finding affordable housing, Landlord/Tenant issues, health-care issues, prevention strategies, etc.
  • Participates on the Outreach Services Team in order to receive feedback and assignments, communicate department information, build organizational unity, and in order to collaborate with Team Members.
  • Collaborates with other Swope Health Services Providers in order to provide continuity of care, locate clients for follow-up, and to ensure client progress in regard to goals and objectives of the Treatment Plan and Program, including mental health treatment, medical appointments, medication compliance, recovery issues, legal issues, permanent housing, continuing education, employment, and spiritual support and growth.
  • Completes all required training and cultivates personal and professional development in order to remain current in theory and evidence-based practice, research, and policy, ensuring quality practice and cultural competence.
  • Networks and collaborates with other agencies and advocates for social issues while representing Swope Health Services in the greater Kansas City Community. This includes attending community meetings as assigned and agreed upon by supervisor and the Outreach Services Team.
  • Attends individual and/or group supervision to formally evaluate practice with clients, improve services and effectiveness with clients, and discuss concerns.
  • Completes all other duties as assigned.
