About The Position

The Outreach Case Manager is responsible for providing direct support, outreach, advocacy, and case management services to youth and young adults ages 18-24 experiencing homelessness or housing instability. This position works closely with individuals in the community to build trusting relationships, assess immediate and long-term needs, and connect participants to critical resources including housing, employment, education, healthcare, mental health services, and life skills support. The Outreach Case Manager conducts street outreach, community engagement, workshops, opioid education and awareness, crisis intervention, and ongoing case management to help youth achieve stability, self-sufficiency, and personal goals. The ideal candidate is compassionate, trauma-informed, culturally responsive, and committed to empowering vulnerable youth populations through strengths-based support and advocacy. This role requires strong communication skills, the ability to work independently in community settings, and experience engaging high-risk or underserved populations in a respectful and non judgmental manner.

Nice To Haves

  • Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Communications, or related field desired but not a requirement
  • Experience with outreach, case management, mentoring, or community engagement preferred
  • CPR/First Aid certification
  • Bilingual skills are a plus
  • Personal or lived experience with the criminal/legal system, housing instability, social services, or other systemic challenges is valued.

Responsibilities

  • Conduct outreach to areas frequented by youth ages 18-25 - schools, community centers, libraries, encampments.
  • Administer assessments (VISPDAT) and refer youth to county's Homeless Management Information System
  • Lead tabling events, workshops and collaborate with community partner organizations
  • Build positive relationships with participants and provide ongoing support and encouragement
  • Utilize Motivational interviewing skills to engage young adults to participate in Drop In Center Services
  • Connect youth to services and resources including education, opioid prevention, job readiness, housing, healthcare, and mental health support
  • Facilitate workshops, support groups, and community engagement activities
  • Maintain accurate participant records, case notes, and outreach data
  • Collaborate with community partners, schools, employers, and service providers
  • Support youth through case management in setting goals and accessing opportunities for growth and development
  • Attend agency and other program meetings; including community meetings and trainings as appropriate.
  • Must be able to communicate effectively, get along with co-workers, management, and deal effectively and professionally under pressure.
  • Transport clients to appointments, meetings, etc. as needed.
  • Participate in community events, outreach campaigns, and networking opportunities
  • Regular attendance is a requirement of the position
  • Advocate for youth needs and help create a safe, inclusive, and supportive environment
