About The Position

The Managed Care Outreach Coordinator supports patient engagement, retention, and quality outcomes for our managed care population. This position focuses on outreach to newly assigned patients, patients overdue for care, and recently termed members, with the goal of increasing access, improving retention, and supporting quality initiatives. The role is responsible for creatively engaging hard-to-reach patients and helping fill provider schedules by targeting patients due for services like annual physicals and wellness exams. The ideal candidate is bilingual in Spanish and English, with strong communication skills and a passion for delivering patient-centered care in a diverse, community-based setting.

Requirements

  • Successful completion and acceptable results of post offer physical examination, drug screening, and criminal background check
  • Requirements for vaccinations and preventative testing: Influenza, TB, Covid-19
  • High school diploma or equivalent; associate degree or higher preferred.
  • Fluent in both Spanish and English (spoken required and written preferred).
  • 1–2 years of experience in a healthcare, community outreach, or customer service role.
  • Experience communicating with diverse populations.
  • Strong computer skills including EHR systems (e.g., eClinicalWorks), Microsoft Office, and outreach tools.

Nice To Haves

  • Experience in a Federally Qualified Health Center (FQHC) or safety-net healthcare setting.
  • Familiarity with Medi-Cal, Medicare, and managed care plans.
  • Understanding of population health, quality measures, or value-based care.

Responsibilities

  • Conduct outreach (calls, texts, emails, and mailers) to welcome and schedule newly assigned patients.
  • Re-engage patients who have not been seen in over 12 months.
  • Contact termed members to support re-enrollment or continuity of care.
  • Engage patients in both English and Spanish, tailoring communication to cultural and linguistic needs.
  • Monitor provider schedules for availability.
  • Fill open appointments by targeting patients who are due for care or services.
  • Coordinate with front office and clinical teams to ensure efficient scheduling.
  • Use care gap lists and registries (e.g., annual physicals, AWVs, screenings) to drive outreach.
  • Support compliance with UDS measures and managed care quality metrics.
  • Develop innovative strategies to reach patients who are unresponsive to traditional outreach methods.
  • Lead mailing campaigns, reminders, and educational communications.
  • Partner with other departments to address patient barriers to care and boost compliance.
  • Work closely with care teams, enrollment staff, and call center agents.
  • Track and document outreach attempts, patient responses, and outcomes.
  • Assist with performance dashboards and reports for internal stakeholders.
