About The Position

The Outreach Coordinator is responsible for planning and implementing marketing and sales activities for Westminster Canterbury at Home programs. Major functions include developing and maintaining referral sources, service coordination, promoting and communicating the image and philosophy of Westminster Canterbury at Home, sales support to programs, and referral of clients to appropriate and desired services. Requirements

Bachelor’s degree in a related field or active, Virginia RN or LPN license.
Minimum of 1 year experience in a Home Health or Hospice environment.
Minimum of 1 year experience in marketing or outreach.
Valid Virginia Driver's License and Proof of Insurance.
Motor Vehicle Driving Record satisfactory to Westminster-Canterbury and its insurance providers.
Flu-Vaccinated.

Nice To Haves

Experience in marketing activities specifically in the home health environment.
2+ years of sales experience.
COVID-19 Vaccinated. As of now, the 2025–2026 COVID-19 vaccine formulation is being distributed to providers and pharmacies; however, it has not yet received official approval from the CDC. In Virginia, pharmacists may administer COVID-19 vaccines under the Virginia Board of Pharmacy’s statewide protocols or with a direct order from a licensed medical provider. We encourage all new hires to consult with their healthcare provider regarding the most current vaccination recommendations. Please be aware that, depending on future CDC guidance or regulatory updates, we may require proof of the most recent COVID-19 vaccination as part of our ongoing commitment to health and safety.

Responsibilities

Build and maintain strong relationships with key referral sources to generate new Home Health and Hospice referrals.
Develop new referral pipelines and follows up with discharged clients to determine their need to re-establish services.
Provide referral partners with up‑to‑date information on programs, services, and admission criteria.
Coordinate referrals and intakes, including pre‑admission reviews and obtaining physician orders when needed.
Achieve individual sales and volume goals for both Home Health and Hospice, aligned with market strategy.
Identify needs within the senior community and coordinate educational outreach and program offerings.
Collaborate with the Outreach Director to develop the annual Marketing Plan and participate in ongoing competitive analysis, referral tracking, and market evaluation.
Represent Westminster‑Canterbury at Home at community events, professional meetings, and outreach activities.
Maintain current knowledge of funding sources, regulations, and industry standards for Home Health and Hospice.
Support departmental planning and communication to ensure alignment across programs.
Perform other job duties as assigned.
