About The Position

The Community Outreach Specialist works collaboratively with the Community Outreach Coordinator to develop, implement, and evaluate cancer outreach and community education initiatives. This role serves as a liaison between the Cancer Center and community-based organizations, promoting awareness of cancer prevention, early detection, and available screening services. The Community Outreach Specialist supports educational programming, outreach events, patient navigation activities, and data collection efforts to improve community engagement and healthcare access.

Responsibilities

  • Establish and maintain partnerships with community-based organizations to provide education, information, and referrals related to cancer screening and healthcare services.
  • Collaborate with community organizations to increase awareness of cancer prevention and early detection initiatives.
  • Attend and participate in community events, health fairs, educational programs, and outreach activities as assigned.
  • Organize and support community education programs focused on cancer prevention, health promotion, and access to care.
  • Assist in identifying and utilizing culturally and linguistically appropriate educational materials for diverse populations.
  • Promote hospital and cancer center services within the community while ensuring community resources are available to patients and staff.
  • Support evaluation efforts by collecting and tracking outreach data to assess program effectiveness and identify opportunities for growth.
  • Assist with patient navigation activities, including attending screening events, supporting patient registration, and facilitating follow-up appointments.
  • Contact patients following screenings to assist with scheduling appointments and connecting them with appropriate healthcare providers.
  • Coordinate and facilitate appointments and referrals as appropriate to support continuity of care.
  • Assist in identifying geographic regions and underserved populations that may benefit from outreach services.
