About The Position

The Outreach & Engagement Specialist plays a vital role in helping individuals with opioid use disorders remain connected to life-saving treatment and supportive services. This position is ideal for someone who is passionate about meeting people where they are, building trust, and removing barriers to care. You’ll join a compassionate, mission-driven team dedicated to supporting recovery, strengthening engagement, and improving outcomes for individuals receiving Medication Assisted Treatment (MAT). In this role, you will provide consistent outreach, education, and support to clients who may not be fully engaged in outpatient treatment services. Work may occur remotely, onsite, or in community settings, and includes individual outreach, group facilitation, documentation, and collaboration with multidisciplinary treatment teams. Your efforts will directly contribute to increased access to care, sustained recovery, and improved quality of life for those served.

Requirements

  • Certified Peer Specialist (CPS) AND Community Support Specialist through the State of work.
  • A bachelor’s degree from an accredited college or university in a human services related field (social work, psychology, nursing, education, etc) OR An individual with any four year degree and two years qualifying experience, OR An Individual with any four year combination of higher education and experience, OR An individual with a High School Diploma or equivalent and a minimum of four years of qualifying experience.
  • Certification as a Peer Specialist by the state or meeting minimum requirements necessary to obtain certification as peer specialist within 6months of hire.
  • Be at least 21 years of age.
  • Self identify as a present or former client of mental health or substance use services OR self identifies as a person in recovery from mental health or substance use disorder.
  • Complete a training program and if required, pass a standardized examination.

Responsibilities

  • Maintain an assigned caseload of clients receiving Medication Assisted Treatment who may require additional engagement and support
  • Conduct regular outreach with clients through phone, virtual, and face-to-face contact
  • Meet with clients in the community to provide resources, education, and harm-reduction services such as Narcan distribution
  • Facilitate a minimum of two morning and two afternoon or evening group sessions focused on education and support for MAT participants
  • Build rapport with clients to reduce barriers to treatment and increase participation in services
  • Accurately document outreach activities, individual contacts, and group sessions in the electronic health record in accordance with DMH, CARF, and CSTAR standards
  • Update treatment plans to reflect outreach efforts and client participation in support services
  • Collaborate with treatment teams across locations by providing client updates and contributing to treatment recommendations
  • Participate in scheduled individual supervision and team meetings
  • Identify opportunities for program improvement with a focus on client needs
  • Represent the organization professionally and support organizational goals and objectives
  • Perform other duties as assigned by leadership
