The Outpatient Pharmacist practices within a high-volume pharmacy located in a Family Health Center connected to the hospital, supporting approximately 550 prescriptions per day and ensuring seamless transitions from inpatient to outpatient care. This role is responsible for verifying and dispensing medications, conducting clinical order review, and maintaining a strong focus on accuracy and patient safety. Pharmacists also support prior authorization workflows during designated remote work periods, with the opportunity for one remote week per month. Working in a collaborative, patient-centered environment, this role emphasizes autonomy, efficiency, and continuity of care while supporting a diverse patient population. A caregiver in this position will work rotating shifts, Monday through Friday from 8:00am to 8:00pm with weekend rotations every few months with no Sunday routine shifts. Caregivers will be able to work from home 1 week every 4 weeks.

Requirements

  • PharmD or Bachelor’s of Science in Pharmacy from an accredited College of Pharmacy
  • Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA)
  • Completion of a Board of Pharmacy approved course in the administration of immunizations within 90 days of hire for those administering immunizations

Nice To Haves

  • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience
  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area
  • Board Certification

Responsibilities

  • Provide pharmacy services and ensure the appropriate selection, dosing, administration and dispensing of medication orders.
  • Provide safe, efficient, patient-centered, evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Contribute to medication safety initiatives.
  • Document services in a complete, accurate and timely manner.
  • Ensure all pharmacy operations comply with national accreditation standards.
  • Maintain professional interactions with patients to facilitate successful patient experiences.
  • Educate and train students, residents, other caregivers, and/or patients.
  • Be accountable for drug use policy adherence.
