This role involves planning, developing, and providing diet interventions, medical nutrition therapy, nutrition care, and preventive counseling to individuals and groups in various settings including primary and specialty patient/resident care, acute care, skilled nursing, out-patient, and community settings. The dietitian will provide medical nutrition therapy services and monitor the nutritional care of clients/patients/residents of all ages, from neonatal to geriatric. This includes nutritional assessment, individualized care plans, provider order recommendations, implementation of nutrition therapy (including verbal orders, diet modification, nutrition support, oral nutrition supplements), education and counseling for disease and nutrition risk management, and evaluation of outcomes. The role also focuses on optimizing self-management education and skills for chronic conditions, recognizing and providing for nutritional needs based on cultural, ethnic, and religious beliefs, and evaluating treatment effectiveness using an evidence-based approach. The dietitian will also work on nutrition care protocols, act as a leader for the healthcare team in implementing nutrition therapies, educate on diets according to disease state and health risk, incorporate new trends and techniques, tailor clinical education materials, perform quality control, maintain infection control standards, and participate in clinical performance improvement activities and research.
