About The Position

Under the direction of the Cardiovascular Service Line Nurse Director and the CAD Program Coordinator, the Outpatient Cardiac Services Registered Nurse (RN) collaborates within a multidisciplinary team to deliver safe, effective, and patient-centered care. This role supports, guides, and educates patients and their families throughout the surgical process across the continuum of care. The RN assesses patient needs, develops and implements individualized care plans, and evaluates patient responses to achieve desired outcomes. The RN also serves as an advocate, assisting patients and families in navigating the complexities of the healthcare system.

Requirements

  • Graduate of an accredited college or university affiliated nursing program.
  • Current Licensure as a Registered Nurse (RN) in Connecticut.
  • BLS (AHA/ARC).
  • Minimum of 3 years of nursing experience within a cardiovascular surgical setting or related specialty.
  • Basic computer proficiency required.
  • Strong communication, organizational, and critical thinking skills.

Nice To Haves

  • BSN preferred.
  • Experience working with a cardiovascular surgical team preferred.
  • Bachelor of Science in Nursing (BSN) degree preferred.
  • ACLS
  • Experience in outpatient cardiac services, care coordination, or patient navigation strongly preferred.

Responsibilities

  • Assess, plan, implement, and evaluate individualized patient care plans
  • Educate patients and families on procedures, recovery expectations, and ongoing care needs
  • Serve as a patient advocate and care navigator throughout the surgical continuum
  • Communicate effectively with interdisciplinary team members, patients, and families
  • Support quality improvement initiatives that enhance patient outcomes
  • Utilize electronic medical record systems and clinical documentation tools
  • Demonstrate teamwork through effective problem solving, communication, and conflict resolution
