About The Position

The Transplant Coordinator oversees and coordinates the care of heart transplant patients throughout the pre- and post-transplant journey. This role serves as the primary liaison between patients, families, physicians, surgeons, and multidisciplinary care teams to ensure seamless evaluation, education, treatment planning, and ongoing support.

Requirements

  • Associates Degree
  • 1-2 years of previous job-related experience
  • Three years of professional nursing experience
  • CPR (Cardiopulmonary Resuscitation)
  • Current Registered Nurse License State of Illinois

Nice To Haves

  • Bachelor's Degree in Nursing
  • 3-5 years of previous job-related experience
  • At least 1 year of previous transplant, outpatient, or dialysis experience highly desirable
  • Specialty certification in clinical or functional area of nursing granted by a national nursing organization
  • National Certification as Transplant Coordinator or other related specialty

Responsibilities

  • Managing transplant evaluations
  • Reviewing diagnostic and laboratory results
  • Coordinating patient care and medication management
  • Maintaining transplant waitlist information with UNOS
  • Ensuring compliance with regulatory and insurance requirements
  • Educating patients and families on all aspects of the transplant process
  • Providing clinical support and triage
  • Facilitating communication among stakeholders
  • Supporting quality improvement initiatives
  • Maintaining accurate medical records
  • Preparing reports and documentation for regulatory agencies
  • Collaborating across departments to deliver high-quality transplant care
