As an Operation Home Case Manager, you'll work directly with individuals and families experiencing homelessness, chronic housing instability, and other barriers to stable housing. Your role goes beyond helping someone find a place to live - you'll walk alongside clients as they work toward maintaining housing, increasing stability, building independence, and creating a path toward long-term self-sufficiency. You'll manage a caseload of approximately 20–25 clients, developing individualized plans based on each household's unique needs and goals. You'll connect clients with community resources, advocate for benefits and supportive services, help navigate housing searches, collaborate with landlords and community partners, and provide ongoing support once housing is secured. This is a relationship-driven role for someone who can balance compassion with accountability. You'll celebrate progress with your clients while also helping them navigate setbacks, overcome barriers, develop practical life skills, and stay focused on the goals they've created for themselves. Not every day will look the same. You may spend part of your day completing case notes and coordinating services, then head into the community for a home visit, connect with a landlord, collaborate with another provider, or help a client navigate an unexpected crisis.
