About The Position

The Nurse Navigator functions within the multidisciplinary team as an advocate, educator, and counselor for referred oncology patients with selected diagnoses, with an emphasis on patients diagnosed with rectal cancer. The Navigator is responsible for ensuring all patients referred with a cancer related diagnosis or suspicious finding receive effective and comprehensive services.

Requirements

Bachelor's degree in nursing.
ONCC certification required within 18 months of hire date. Ongoing certification is required.
Minimum three years oncology experience required, with a strong knowledge base related to common types of cancers and treatment.
Experience with common computer programs

Responsibilities

Serve as the primary nurse navigator for rectal cancer patients, ensuring timely diagnostic assessments and smooth coordination of treatment plans.
Assist with the scheduling and coordination of patient appointments across specialties, ensuring efficient use of resources and a streamlined patient experience.
Provide patient education on diagnosis, treatment options, and care plans, acting as a resource for patients and families throughout their care journey.
Conduct patient outreach and follow-up to ensure continuity of care and address any patient concerns or barriers to treatment.
Oversee the coordination and facilitation of all tumor boards, ensuring the right multidisciplinary team is assembled for each case.
Manage meeting logistics, including minute-taking, tracking action items, and ensuring compliance with continuing medical education (CME) reporting.
Collaborate with physicians, surgeons, oncologists, and other clinical staff to ensure timely and accurate case presentations.
Lead the program’s efforts to maintain and achieve accreditation from the National Accreditation Program for Rectal Cancer (NAPRC).
