About The Position

The Oncology Nurse Navigator (ONN) serves as a dedicated clinical partner for members navigating a cancer diagnosis. This role provides personalized education, care coordination, and ongoing support throughout the treatment and survivorship journey. The ONN will manage a caseload of members, conduct comprehensive oncology intake assessments, and educate members and families on their diagnosis, treatment options, and available resources using evidence-based guidance. A key aspect of this role is providing fluent, culturally competent oncology navigation support to Spanish-speaking members, conducting clinical calls in both English and Spanish. The ONN will coordinate multidisciplinary care by facilitating communication between members, treating providers, and the broader Lantern Care Coordination (LCC) team. They will also identify and address barriers to care, including psychosocial, financial, and logistical needs, connecting members with appropriate support resources. Accurate and timely documentation within Lantern's care management platform is essential, as are timely outreach, follow-ups, and care coordination tasks. The role involves participating in call auditing processes, collaborating with the Lead ONN, LCSWs, Medical Records Specialists, Care Advocates, and cross-functional partners, and contributing to team meetings and quality improvement initiatives. Adherence to Lantern workflows, clinical protocols, and documentation standards is required.

Requirements

  • Bachelor's degree in Nursing (BSN)
  • Active RN license in a compact state with unencumbered ability to be licensed in all 50 states
  • Minimum 3 years of oncology nursing experience
  • Active ONCC certification in Oncology Nursing (OCN) with unencumbered ability to maintain certification throughout employment
  • Fluent bilingual proficiency in English and Spanish, with demonstrated ability to conduct clinical conversations in both languages
  • Proficiency in Microsoft Outlook and Microsoft Office Suite
  • Familiarity with NCCN guidelines and ClinicalTrials.gov
  • Ability to work independently in a hybrid environment; must be Dallas area based

Responsibilities

  • Manage a full, active caseload of members with cancer diagnoses, providing individualized navigation, education, and clinical support from diagnosis through survivorship.
  • Conduct comprehensive oncology intake assessments to establish member needs, goals, and care priorities.
  • Educate members and families on diagnosis, treatment options, clinical pathways, and available resources using evidence-based, NCCN-aligned and ONCC-aligned guidance.
  • Provide fluent, culturally competent oncology navigation support to Spanish-speaking members, conducting clinical calls in both English and Spanish.
  • Coordinate multidisciplinary care by facilitating communication between members, treating providers, and the broader LCC team.
  • Identify and address barriers to care including psychosocial, financial, and logistical needs; connect members with appropriate support resources.
  • Complete accurate, timely documentation of all member interactions and clinical activities within Lantern's care management platform.
  • Complete all outreach, follow-ups, and care coordination tasks within defined timeframes.
  • Participate in call auditing processes and consistently apply feedback to maintain and improve quality of care delivery.
  • Collaborate closely with the Lead ONN, LCSWs, Medical Records Specialists, Care Advocates, and cross-functional partners.
  • Contribute to team meetings, case reviews, and quality improvement initiatives.
  • Adhere to Lantern workflows, clinical protocols, and documentation standards at all times.
