About The Position

At Atlas Oncology Partners, we believe every patient deserves personalized support as they navigate the complex cancer care system. We are seeking a dedicated Oncology Care Coordinator to serve as a consistent, non-clinical point of contact for patients and families throughout the cancer journey. This role is designed to reduce barriers to care, improve access to timely services, and provide practical and emotional support through close coordination with patients, providers, and interdisciplinary team members. The Oncology Care Coordinator ensures no patient faces cancer alone. As a small nimble team, this role will support vaccinations, in addition to care coordination.

Requirements

  • Bachelor’s degree
  • Strong communication and interpersonal skills, including empathy and cultural sensitivity
  • Proficiency in EMR use, phone communication, and task tracking systems
  • Comfort with change, process improvement, and working in a start-up or growing organization

Nice To Haves

  • Bachelor’s degree in social work, Public Health, Nursing, Health Education, or a related field
  • Oncology Patient Navigator Certification
  • Experience in phlebotomy or Medical Assistant License
  • Experience in patient navigation, care coordination, oncology, or a healthcare support role
  • Experience working with diverse populations and addressing social determinants of health
  • Detail-oriented and highly organized; able to manage multiple patient caseloads and prioritize tasks
  • Experience advocating for patients and helping them access financial, transportation, or housing assistance
  • Comfortable speaking with patients by phone or in person to provide guidance, reminders, and support
  • Team player who thrives in fast-paced, mission-driven healthcare environments

Responsibilities

  • Serve as a primary point of contact for patients and families, helping them navigate appointments, treatment, and care coordination
  • Help connect patients to accurate, up-to-date educational resources regarding their diagnosis and treatment, as directed by the clinical care team.
  • Screens for potential barriers to care and collaborates with appropriate team members to address logistical, financial, cultural, and psychosocial needs
  • Assist with scheduling and follow-up appointments across the oncology continuum (e.g., surgery, infusion, radiation)
  • Support connection to support groups, counseling, social work, and other internal or community-based services
  • Establish trusting relationships with patients and caregivers through regular outreach and touchpoints
  • Participate in interdisciplinary meetings and escalate patient concerns or needs appropriately
  • Ensure patients complete key milestones, such as diagnostics, consults, and treatment starts
  • Document patient interactions and support activities in the EMR clearly and in a timely manner
  • Work closely with RN Care Coordinators, Social Workers, and Clinical Teams to ensure seamless, wraparound care
  • Support in phlebotomy and vaccinations, as needed
