This position provides skilled nursing care, assessment, support, and crisis intervention for hospice patients during the weekend hours. The role is responsible for responding to patient and family needs, coordinating care, managing symptoms, and providing education and emotional support to patients facing a terminal illness. The On-Call RN works collaboratively with the IDG team to ensure high-quality, compassionate end-of-life care while promoting patient comfort, dignity, and quality of life. This position is paid hourly, non-exempt, eligible for overtime. The ideal candidate is empathetic, warm, compassionate, a great listener, a strong communicator, respectful, professional, and can exercise independent and sound judgment to assist patients with complex and diverse issues effectively and efficiently.

Requirements

  • Graduate from an accredited school of nursing.
  • Currently a Georgia licensed Registered Nurse (RN).
  • Minimum of 2 years of recent bedside nursing experience (Med-Surg, ICU or CCU preferred).
  • Hospice experience a plus, minimum of 1 year continuous work experience.
  • Current valid driver's license.
  • Auto liability insurance and your own reliable transportation.
  • Strong clinical assessment and critical-thinking skills.
  • Knowledge of hospice philosophy, symptom management, and end-of-life care principles.
  • Ability to manage urgent situations calmly and effectively.
  • Excellent communication and interpersonal skills.
  • Acceptance and adaptability to different social, racial, cultural and religious modes.
  • Ability to work independently with minimal supervision.
  • Experience or comfortable with pronouncing patient death.
  • Proficiency with electronic health record documentation systems.

Responsibilities

  • Respond to weekend patient calls, assessing medical needs and determining appropriate interventions.
  • Provide skilled nursing visits to hospice patients in homes, assisted living facilities, nursing facilities and other care settings as needed.
  • Assess patient condition and identify changes requiring intervention or physician notification.
  • Provide advice on symptom management using pharmacological and non-pharmacological measures.
  • Answer questions and provide education regarding patient's condition and placing courtesy calls to follow up on patients post intervention.
  • Assist with medication orders, DME and faxing documents/orders as needed.
  • Develop and re-evaluate the patient/family care plan to meet their needs and maintain continuity of care.
  • Document visits in electronic medical record system.
  • Participate in post admission, deaths and continuous care visits as assigned.
  • Support bereaved families and caregivers during crisis situations and at the time of death.
  • Perform specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor's orders.
  • Participate in hand-off communication with weekday clinical staff to ensure continuity of care.
  • Follow the policies and procedures of Hospice.
  • Observe confidentiality and safeguard all patient-related information in compliance with the HIPAA regulations.
  • Comply with infection control policies and protocols.
