About The Position
Luna Del Valle Hospice is seeking a part-time On-Call Licensed Practical Nurse (LPN) to join their team. This role involves delivering direct patient care in the home setting under the supervision of a Registered Nurse (RN). The position is crucial for ensuring high-quality, compassionate end-of-life care that respects patient dignity and supports families during a vulnerable time. The company is nurse- and healthcare professional-owned, locally operated in Albuquerque, NM, with a mission to provide dignified, compassionate, and holistic end-of-life care across New Mexico.
Requirements
- Graduate of an accredited Licensed Vocational/Practical Nursing Program
- Current and valid LPN license in the State of New Mexico
- CPR certification
- A valid driver’s license with auto insurance
- At least 1 year of nursing experience
- Strong organizational, time management, and communication skills
- A cooperative, compassionate approach to patient care
- Ability to handle emotional demands with grace and professionalism
- Basic math skills for medication and treatment administration
Nice To Haves
- Hospice experience preferred
Responsibilities
- Administer medications and treatments per Hospice policies and clinical standards
- Perform baseline nursing skills with clinical competency and in accordance with state practice acts
- Monitor patient condition and promptly report any changes, needs, or concerns to the supervising RN
- Assist in developing and updating patient-centered Plans of Care, including measurable goals and interventions
- Maintain accurate and timely documentation of nursing interventions, observations, and education provided
- Provide emotional support to patients and families, always upholding patient rights and dignity
- Communicate effectively with physicians, interdisciplinary team members, patients, and families
- Participate in team conferences and contribute to collaborative care planning
- Promote teamwork by supporting and assisting fellow staff members when needed
- Maintains patient confidentiality and adheres to HIPAA and privacy policies
- Participates in continuing education and in-service trainings
- Keeps current with health care regulations and standards
- May be required to participate in occasional on-call rotation
