About The Position

We are seeking an experienced Office LPN to join our team at University Home Care Inc in Livonia, MI. The ideal candidate will have a background in Skilled Medicare HomeCare, with hands-on experience working in a home care office setting using skilled Kinnser software. This role requires a strong understanding of nursing visit frequencies and scheduling for nursing, PT, and OT visits. You will play a key role in managing referrals, ensuring all necessary documentation is collected to convert referrals into active patients, verifying eligibility, obtaining prior authorizations, and entering patient information into the system. Additionally, you will coordinate and staff cases, follow up on doctors' orders, and provide support as the right hand to the Director of Nursing, helping to manage the department efficiently. Occasional urgent visits to patient locations may be required.

Requirements

  • Licensed Practical Nurse (LPN) with experience in a Skilled Medicare HomeCare setting.
  • Proficient in using Kinnser software or similar home care office systems.
  • Strong knowledge of nursing visit frequencies and homecare documentation standards.
  • Ability to manage referrals, eligibility checks, and prior authorization processes.
  • Excellent organizational and communication skills.
  • Capability to work independently and support nursing leadership in a managerial capacity.
  • Must be able to occasionally perform home visits on short notice.

Responsibilities

  • Manage and coordinate nursing, PT, and OT visit scheduling based on established frequencies.
  • Process hospital referrals by collecting and verifying all required documentation for patient admission.
  • Check patient eligibility and secure necessary prior authorizations.
  • Enter patient data accurately into the computer system.
  • Staff cases appropriately to ensure timely and quality care delivery.
  • Follow up on physicians’ orders and ensure compliance with care plans.
  • Assist the Director of Nursing with department management and administrative tasks.
  • Occasionally conduct urgent home visits as needed.
