Office LPN - Family Medicine - Dennison
About The Position
Avita Health System is seeking a Licensed Practical Nurse to join the Health Services Department at its Bucyrus location, serving the Family Medicine clinic in Dennison. Under the supervision of a physician or Allied Health Provider (CNP/PA), the LPN assesses, plans, and implements patient nursing care within the physician clinic. The position includes patient and family education, patient advocacy aligned with the facility’s mission, and coordination with clinical staff on patient flow and clinical operations as directed by the physician. The schedule is Monday through Thursday, 8 a.m. to 5 p.m., and Friday, 7 a.m. to 4 p.m., for 80 hours per pay period. Avita Health System serves Crawford and Richland counties through three hospitals and numerous clinic locations, employing more than 2,300 team members and over 200 physicians and advanced practitioners.
Requirements
- Current Licensed Practical Nurse license in the State of Ohio.
- Current BLS certification.

- Two or more years of previous experience in a physician office.
Responsibilities
- Assess, plan, and implement nursing care for patients in the physician clinic.
- Participate in patient and family teaching.
