About The Position

The client is seeking an Occupational Therapy professional for a position in Chandler, Arizona, working a 5x8 day shift. Requirements

• Current state licensure
• At least 1 year of recent experience
• Current resume
• Current BLS and/or ACLS and/or specialty certifications
• Two current clinical references
• Physical examination completed within the past 12 months
• TB skin test completed within the past 12 months
• MMR, Hepatitis B, and Varicella titers
• Respiratory fit test completed within the past 12 months
• Current Tdap and flu vaccinations
• Must pass pre-employment screening, a background check, and urine drug screening
