About The Position
FOX Rehabilitation is hiring Occupational Therapists to provide one-on-one care to older adults in private residences and senior living communities in Chattanooga, Tennessee, and surrounding communities. Clinicians help patients improve mobility, independence, wellness, and quality of life through the Geriatric House Calls™ model. Care may also include telehealth and other virtual services when appropriate. The role offers autonomy to create and manage a treatment schedule, build ongoing patient relationships, and work in home and community settings. Clinicians collaborate with an interdisciplinary team and may have opportunities to mentor, teach, supervise assistants, make patient referrals, and contribute written or social content. FOX offers full-time, part-time, and PRN/Fox Flex employment pathways; new graduates may be eligible for its Emerging Professionals Mentor Program.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Ability to wear personal protective equipment when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
