Occupational Therapist - Franklin, TN
About The Position
FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one care to older adults in private residences and Senior Living Communities in Franklin, Tennessee, and surrounding communities. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may also include telehealth and other virtual services when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work across in-home and senior living settings. FOX describes its practice as clinician-operated, evidence-based, relationship-focused, and committed to abolishing ageism. Full-time, part-time, and PRN/FOX Flex pathways are available, with benefits eligibility depending on role and, for part-time positions, minimum-hour requirements. New graduates may be eligible for the Emerging Professionals Mentor Program.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to wear personal protective equipment when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
