About The Position
Ascend Home Health & Hospice is seeking a compassionate, skilled PRN Occupational Therapist to provide personalized, patient-centered care in patients’ homes. The role focuses on helping patients maintain or improve independence, safety, and quality of life while working closely with patients, families, and the interdisciplinary care team. The service area is Pocatello, and the setting is Home Health & Hospice.
Requirements
- Graduate of an accredited Occupational Therapy program.
- Current state OT license in good standing.
- Strong communication, organization, and clinical assessment skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation and a valid driver’s license.
- Current CPR certification or ability to obtain it, as required.
Nice To Haves
- Previous home health or hospice experience preferred.
Responsibilities
- Evaluate patients’ functional abilities and develop individualized treatment plans.
- Provide occupational therapy services to improve daily living skills, mobility, safety, and independence.
- Recommend adaptive equipment and home modifications as needed.
- Educate patients and caregivers on exercises, safety strategies, and techniques that support daily activities.
- Collaborate with physicians and the interdisciplinary team to coordinate patient care.
- Complete accurate and timely clinical documentation in accordance with company policies and applicable regulations.
- Follow individualized plans of care and communicate changes in patient status to the care team.
