About The Position

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full-time, outpatient opportunity. This role involves performing home health visits for adult and geriatric patients in independent and assisted living communities. As an OT, you will be a key member of a multidisciplinary healthcare team, providing rehabilitative care and skilled nursing to help clients remain at home. The position is based out of the Wilmington Senior Living Office and services the North Wilmington/Hockessin area.

Requirements

A current DE Occupational Therapist license.
A minimum of one year of recent work experience as an Occupational Therapist.
Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA (indicated by school transcript or diploma).
Ability to work independently and manage time effectively.
Strong interpersonal skills, organizational skills, and problem-solving skills.
Solid computer skills.

Nice To Haves

Prior home care experience preferred.
Prior experience with electronic medical records (EMR) preferred.

Responsibilities

Perform outpatient therapy evaluations to develop care plans for patients in senior living communities or group homes for adults with intellectual disabilities.
Utilize current research-based approaches to maximize client functional independence.
Collaborate with home health therapy partners to transition care from home health to outpatient services, achieving client-centered goals within their living environment.
Educate clients, families, and community caregivers on care plans to foster a team approach for maximizing functional independence.
Perform evaluations, treatments, and discharge planning for clients.
Document patient care and treatment approaches using the outpatient EMR platform.
