About The Position
Innovate Rehab and Wellness is seeking a licensed PRN Occupational Therapist to work in a skilled nursing facility. The therapist evaluates patients’ functional abilities and therapy needs, creates and delivers individualized treatment plans, and helps patients improve motor skills, coordination, and daily function so they can regain independence and quality of life. The role emphasizes one-on-one patient care, with group or concurrent treatments accounting for less than 1% of treatment, and a flexible, as-needed schedule intended to support work–life balance. Pay is a competitive hourly rate based on experience. Requirements
- Master’s or doctorate in Occupational Therapy from an accredited program.
- Valid state occupational therapist license.
- Strong communication and problem-solving skills.
- Passion for patient-centered care.

- CPR certification.
Responsibilities
- Conduct thorough occupational therapy evaluations.
- Develop and adjust personalized treatment plans.
- Use therapeutic techniques and adaptive equipment.
- Train patients and caregivers on exercises and assistive devices.
- Educate patients on strategies to improve function and safety.
- Collaborate with physical therapists, speech-language pathologists, physicians, and the full care team.
- Accurately document patient progress and treatment changes.
- Stay current with occupational therapy best practices and research.
