About The Position
Ultimate Therapy is seeking an Occupational Therapist (OT) to join the rehabilitation team at Autumn Lake at Salem County. This role offers the opportunity to help residents regain independence and improve everyday function while working alongside a supportive interdisciplinary team in a skilled nursing and post-acute care setting.
Requirements
- Active New Jersey Occupational Therapist license required
- Graduate of an accredited Occupational Therapy program
- Strong clinical, communication, and organizational skills
Nice To Haves
- Skilled nursing or rehabilitation experience preferred, but not required
- New graduates are welcome to apply
Responsibilities
- Evaluate residents and develop individualized occupational therapy treatment plans
- Provide skilled interventions focused on activities of daily living (ADLs), strength, coordination, balance, cognition, and functional mobility
- Help residents improve independence with everyday tasks and routines
- Recommend adaptive equipment and educate residents and caregivers on proper use
- Monitor progress and adjust treatment plans based on resident needs
- Complete accurate and timely clinical documentation
- Collaborate with PT, SLP, nursing, and other members of the care team
