About The Position

HealthPro Heritage is seeking a full-time Occupational Therapist to join its collaborative rehabilitation team in Fork, South Carolina. The role focuses on helping older adults maintain independence, regain function, and improve their quality of life. The therapist will evaluate clients’ physical, emotional, and cognitive abilities; create personalized treatment plans; educate clients and families about daily activities, adaptive equipment, and environmental modifications; maintain accurate documentation; and advocate for clients’ needs. The position is part of a therapist-led organization providing clinical services across nursing facilities, retirement communities, hospitals, home care, and pediatric schools and clinics.

Requirements

Degree in Occupational Therapy from an accredited institution.
Valid state licensure as an Occupational Therapist, or eligibility to obtain a license.
Strong clinical assessment and diagnostic skills.
Proficiency in developing and implementing effective treatment plans.
Excellent communication and interpersonal skills with a patient-centered approach.
Ability to lift and assist patients as needed.
Ability to stand for extended periods and perform physical tasks related to therapy.

Responsibilities

Evaluate clients’ physical, emotional, and cognitive abilities using observations and standardized tools.
Create personalized treatment plans that meet clients’ goals and enhance their functional abilities.
Educate clients and their families on improving daily activities, including the use of adaptive equipment and environmental modifications.
Maintain accurate records of evaluations, progress, and treatment plans in compliance with legal and ethical standards.
Advocate for clients’ needs and support them in accessing community resources, services, and accommodations.
