About The Position

We are seeking an Occupational Therapist (OT) to join our Home-Based Hospital Care Team! You will deliver acute, hospital-level care to patients in their homes using an evidence-based and patient-centered therapy approach. Help patients regain independence, improve daily function, and safely navigate their home setting, reducing hospital stays and improving outcomes. Work in collaboration with the interdisciplinary team to ensure that patients receive exemplary care in their home.

Requirements

• Bachelor's Degree in Occupational Therapy Required
• Licensure as an Occupational Therapist in the State of Wisconsin Upon Hire Required
• Basic Life Support (BLS) certification within 3 months Required

Nice To Haves

• 2 years prior relevant experience Preferred
