About The Position
FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one care to older adults in Waterbury, Connecticut, and surrounding communities. Care is delivered in patients’ homes and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, and quality of life through evaluations, individualized plans of care, and evidence-based treatment. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may incorporate telehealth and other virtual care into their schedules. FOX offers full-time, part-time, and PRN/FOX Flex pathways; this posting is for a PRN opportunity compensated per unit. New graduates are eligible for FOX’s Emerging Professionals Mentor Program. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and abolishing ageism. Eligible roles may include benefits. The stated Connecticut full-time base pay range is $70,000–$110,000 annually; part-time and PRN compensation is prorated.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with transfers and bed mobility, repositioning patients, extended standing and walking, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy to support patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
