The Occupational Therapist evaluates, plans, directs, and administers occupational therapy modalities to promote patients’ independence and quality of life. Under a licensed physician’s prescription, the therapist develops individualized plans of care to improve, sustain, or restore independence following injury, illness, disability, or cognitive dysfunction. Treatment may involve life skills, activities of daily living, sensorimotor and social activities, and functional, vocational, homemaking, educational, and recreational activities, selected to suit each patient’s abilities and interests. The role provides direct patient care in the patient’s home or other setting they consider home. The Occupational Therapist works under the direction of the Director or designee, completes standardized OASIS assessments and timely documentation, supports hand-offs and transitions through the care continuum, and coordinates care with the healthcare team, physicians, and community. The therapist coordinates visits according to the plan of care, delegates appropriate tasks, maintains annual competencies, and attends continuing education as applicable. The position also requires effective communication and collaboration with the interdisciplinary team, patients, families, and clinical leadership; participation in case conferences, meetings, and assigned committees; and adherence to regulatory, compliance, and agency requirements. The therapist is expected to be adaptable, prioritize and manage time and resources effectively, and contribute to a positive, team-oriented work environment.
