About The Position
Sobe Innovative Rehab is seeking a licensed Occupational Therapist to provide therapy services to hospice patients in a home setting. The role focuses on helping older adults maintain their independence and quality of life through compassionate, evidence- and comfort-based care. PRN therapists work on an as-needed basis and may choose which assignments to accept, offering flexibility and control over their schedules.
Requirements
- Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
- Active Florida Occupational Therapist (OT) license.
Responsibilities
- Evaluate and treat residents in a home setting.
- Develop and implement individualized treatment plans.
- Collaborate with physicians, families, and the interdisciplinary care team.
- Maintain accurate, timely documentation in the EMR.
