About The Position

Hillcrest Health & Living is looking for a compassionate and clinically strong Occupational Therapist to join our rehabilitation team in a full-time role split between Hillcrest Health & Rehab and Glen Haven Village. This position provides the opportunity to support guests and residents across two care settings while working closely with interdisciplinary teams to promote recovery, safety, independence, and quality of life. Requirements

  • Graduate of an accredited Occupational Therapy program required.
  • Active Occupational Therapist license in Nebraska or a compact state required.
  • Basic Life Support certification through the American Heart Association required.
  • Must be able to speak, read, and write English.
  • Must be able to independently lift up to 35 pounds.
  • Must be able to perform repetitive sitting, bending, standing, walking, reaching, and movement as needed.
  • Must be flexible, calm, and professional in stressful situations.
  • Must be able to work in a fast-paced environment with strong attention to detail.
  • Must be able to utilize technology, clinical systems, and resources efficiently and effectively.
  • Must be able to travel between Hillcrest Health & Rehab and Glen Haven Village as scheduled.
  • Possible exposure to communicable diseases and bodily fluids.

Nice To Haves

  • At least two years of Occupational Therapy experience in long-term care, post-acute care, hospital, home care, or a similar setting preferred.
  • A passion for helping guests and residents improve function, safety, confidence, and independence.
  • Strong evaluation, treatment-planning, and clinical-documentation skills.
  • Strong communication and relationship-building skills with guests, residents, families, providers, and team members.
  • Ability to lead plans of care while collaborating with assistants, rehab techs, and interdisciplinary partners.
  • Strong attention to detail with documentation, safety, care coordination, and follow-through.
  • Ability to manage multiple patient needs while maintaining high-quality care.
  • Ability to stay organized and communicate effectively while supporting two locations.
  • A calm, flexible, and professional approach in changing or stressful situations.

  • Dependable transportation and the ability to travel between assigned locations.
  • Previous long-term care, post-acute care, hospital, home care, or similar experience preferred.

Responsibilities

  • Complete occupational therapy evaluations using appropriate tests and measures.
  • Establish timely, individualized plans of care in compliance with departmental standards and state and federal regulations.
  • Provide treatment interventions that support activities of daily living, functional skills, sensorimotor needs, adaptive strategies, safety, and independence.
  • Help guests and residents improve confidence with daily routines, homemaking skills, recreational activities, social participation, and adjustment to disability.
  • Educate guests, residents, and family members using clear communication, adult-learning principles, and sensitivity to individual cognitive and cultural needs.
  • Complete accurate documentation and charging to support therapy services within facility guidelines.
  • Manage each plan of care through timely interdisciplinary collaboration and physician communication as needed.
  • Plan safe and effective transitions within or outside Hillcrest service lines, including recommended services, equipment needs, and handoff communication.
  • Supervise, collaborate with, and delegate appropriate care to assistants, rehab techs, and other support personnel in accordance with applicable regulations.
  • Identify individuals who may benefit from additional medical or therapy services and complete screens according to facility policy.
  • Develop an efficient and effective patient schedule while considering patient preferences, safety, environment, cognitive needs, and productivity standards.
  • Identify patient and facility risks and take appropriate action related to infection control, safety, documentation, equipment, and patient handling.
  • Coordinate care and communication across Hillcrest Health & Rehab and Glen Haven Village.
  • Participate in professional development, clinical education, presentations, advocacy, and other activities that support the therapy profession.
