About The Position
Eddy VNRA is a 5 STAR agency seeking a Full-Time Occupational Therapist for their Homecare team in the SCHENECTADY Area. This role involves managing a caseload of patients requiring home-based therapy, primarily post-acute or sub-acute, in their homes. Requirements
- NYS Occupational Therapist Licensure
- 1 year Occupational Therapy experience is required
- Valid Driver's License and Reliable Vehicle.
Nice To Haves
- Home care experience preferred but not required
Responsibilities
- Promote/provide Occupational Therapy services in the home to assist the patient in achieving his/her optimal level of functioning.
- Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
- Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es).
- Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians.
