About The Position

Eddy Visiting Nursing and Rehabilitation Association, a member of St. Peter’s Health Partners, is seeking a Weekend Track Homecare Physical Therapist working every Saturday and Sunday - 12 hour day shifts - to join its team of dedicated healthcare providers. This position offers independence, cutting-edge technology, competitive compensation, and a strong focus on safety. The role involves admitting patients requiring home-based therapy, primarily post-acute or subacute, and promoting/providing physical therapy services in the home to assist patients in achieving their optimal level of functioning. The therapist will evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic, and medical conditions, caring for one patient at a time. The agency provides a comprehensive orientation program, mentorship, and strong management support.

Requirements

Graduate of an approved Physical Therapy program with a Bachelor of Science, Master’s degree or Clinical Doctorate of physical therapy degree.
Current NYS PT license and registration
Minimum 1 year of Physical Therapist required.
NYS Clean Driver's License and working vehicle
