About The Position

Requirements

  • Bachelors of Science Degree in Occupational Therapy required.
  • Current State of CT Occupational Therapy license required.

Nice To Haves

  • Master’s Degree in Occupational Therapy or Doctor of Occupational Therapy preferred.
  • Previous experience as an Occupational Therapist preferred.

Responsibilities

  • Independently evaluates patients’ level of function and determines, develops, and implements individualized treatment goals and plans in alignment with the patient/caregiver goals.
  • Determines treatment strategies and delivers Occupational Therapy services that promote health and wellness, prevent injury or disability, improve and develop self-dependency and maintenance, productivity, leisure, perceptual motor function, rest, mental, cognitive physical, sensory-perceptual, psychosocial developmental, and emotional wellbeing in a variety of contexts and environments in order to maximize function in the areas of daily living, leisure/recreation, and productivity/work.
  • Delegates patient care responsibilities to Certified Occupational Therapy Assistant.
  • Performs thorough evaluation of patient’s level of function upon Physician referral.
  • Performs evaluations for upper extremity strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, perceptual-cognitive motor function, and functional abilities of daily living.
  • Recognizes physiological, psychological, and age related changes in the patient.
  • Consults with other Health Care Team members as necessary.
  • Develop an individualized patient centered treatment plan of care.
  • Evaluate care plan on an on-going basis, revising as necessary in accordance with patient’s care needs.
  • Establish measurable goals with the patient/family based on appropriate patient care protocols and critical paths in order to meet patient needs.
  • Notify Physician of any changes in status or progress.
  • Deliver therapeutic techniques that are appropriate to patient’s needs and medical status.
  • Implement, deliver and delegate patient care by utilizing appropriate interventions and clinical decision making.
  • Provides patient and caregiver education regarding treatment, precautions, and equipment use.
  • Completes all documentation including but not limited to: evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan and summary.
  • Requires the timely and accurate documentation and coding of skilled and medically necessary services provided to the patient.
  • Participate in patient transition planning and/or patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care.
  • Help to coordinate patient care with other care providers such as Rehab Therapists, Nurses, Social Workers, and other community resources as related to care transition planning.
  • Delegate patient care tasks and provide clinical supervision for Occupational Therapist students, Rehabilitation Aides and Certified Occupational Therapy Assistants.
  • Maintain a clean, neat work environment for patients and staff including adhering to infection control and safety precautions.
  • Restock supplies as necessary.
  • Maintains clinical proficiency, continuing education units (CEUs), annual health stream, and all education requirements as required by the State of Connecticut licensure.
  • Perform other related duties as required.
