About The Position

Prava Therapy is seeking a Part-Time Occupational Therapist to join their embedded therapy team at The Kensington Sierra Madre. This role focuses on providing meaningful and functional therapy integrated into the community's daily life, rather than traditional senior living therapy. The therapist will work closely with residents, families, caregivers, and community staff to help residents maintain independence, engagement, and connection. The position emphasizes a holistic approach to occupational therapy, focusing on the resident's goals, needs, interests, and daily routines to improve their quality of life.

Responsibilities

  • Provide exceptional, individualized occupational therapy based on each resident's goals, needs, interests, and daily routines.
  • Evaluate and treat residents across a range of functional, cognitive, and physical needs.
  • Develop meaningful plans of care focused on participation, independence, safety, and quality of life.
  • Address ADLs, IADLs, cognition, balance, functional mobility, upper-extremity function, and environmental barriers as appropriate.
  • Help residents participate in the activities and routines that give their days meaning.
  • Recognize changes in function and communicate concerns proactively with the interdisciplinary team.
  • Partner with caregivers and community staff to reinforce strategies beyond the therapy session.
  • Provide education that helps the people around the resident support greater independence.
  • Build strong relationships with residents and families.
  • Collaborate with the broader Prava therapy team and community care team.
  • Document thoughtfully and efficiently using Prava's technology and AI-enabled tools.
  • Measure outcomes that matter—not simply minutes or visits.
  • Help create a therapy culture that is visible, responsive, and integrated into the community.
