About The Position

FOX Rehabilitation is hiring Occupational Therapists to provide one-on-one care to older adults in private residences and senior living settings in and around Janesville, Wisconsin. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, functional independence, wellness, and quality of life. Care may also include telehealth or other virtual services when appropriate. The role offers autonomy to create and manage a treatment schedule, develop lasting patient relationships, and work in a clinician-driven, interdisciplinary practice. Employment pathways described include full-time, part-time, and PRN/FOX Flex positions. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program. Compensation varies by experience and employment type; benefits are available for eligible roles.

Requirements

Valid Occupational Therapy license in the state or states of practice, or eligibility to obtain one.
Degree from an accredited Occupational Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the stated physical demands of in-home and community-based care, including patient assistance and transfers, CPR, lifting, extended standing and walking, stair climbing, and therapeutic activities.
Ability to drive to patient homes and senior living communities.
Wear PPE when required.

Responsibilities

Provide one-on-one occupational therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
Document care in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
May supervise PTA/OTAs, mentor emerging professionals, serve as a Clinical Instructor, or contribute written or social media content for additional compensation.
