Contracted In-Home Occupational Therapist
About The Position
Jukebox Health is seeking an Occupational Therapist to help older adults and individuals with disabilities live more safely and independently at home. The company partners with health plans and combines technology with networks of clinicians, suppliers, and installers to provide personalized home modifications and environmental supports nationwide. The role involves in-home visits and is contracted, with a minimum of 10 hours of weekly availability. Compensation is $120 per completed evaluation, covering travel to and from the client’s home, the evaluation, and documentation. Contractors are paid monthly, within two weeks of the previous month’s end. Extended travel may be paid additionally if discussed and approved in advance. The anticipated start date is immediate; credentialing with partner health plans may take time.
Requirements
- Degree in Occupational Therapy from an accredited program.
- Active, unencumbered Occupational Therapist license.
- At least 2 years of clinical experience.
- Strong interpersonal and clinical reasoning skills.
- Reliable transportation and a valid driver’s license.
- Proficiency with technology and digital documentation.
- Ability to work independently while collaborating with a remote team.
Nice To Haves
- Experience in home health, aging in place, or disability services.
- Experience with home modification work.
- Comfort working with new systems and tools.
Responsibilities
- Conduct in-home assessments and functional evaluations.
- Deliver personalized, evidence-based recommendations for home modifications.
- Educate clients and caregivers on fall prevention and safe mobility techniques.
- Engage clients and families in collaborative care planning.
- Work with the Director of Clinical Operations to share findings and support process improvement initiatives.
- Maintain accurate and timely documentation through the electronic system.
