Contracted In-Home Occupational Therapist
About The Position
Jukebox Health is seeking an Occupational Therapist to help older adults and individuals with disabilities live safely and independently at home. The company partners with health plans to provide personalized home modifications and environmental supports nationwide, combining technology with networks of clinicians, suppliers, and installers. The role involves in-home visits and requires a minimum of 10 hours of weekly availability. The anticipated start date is immediate, and compensation is $120 per completed evaluation. Payment is made monthly, within two weeks after the end of the previous month. Per-evaluation compensation includes travel to and from the client’s home, the evaluation, and documentation; additional payment for extended travel may be approved in advance.
Requirements
- Degree in Occupational Therapy from an accredited program.
- Active, unencumbered occupational therapy license.
- At least 2 years of clinical experience.
- Strong interpersonal and clinical reasoning skills.
- Reliable transportation and a valid driver’s license.
- Proficiency with technology and digital documentation.
- Ability to work independently while collaborating with a remote team.
Nice To Haves
- Experience in home health, aging in place, or disability services.
- Fluency in Spanish, Mandarin, or Cantonese to conduct in-home assessments and communicate directly with members and caregivers.
- Experience with home modification work.
Responsibilities
- Conduct in-home assessments and functional evaluations.
- Deliver personalized, evidence-based recommendations for home modifications.
- Educate clients and caregivers on fall prevention and safe mobility techniques.
- Engage clients and families in collaborative care planning.
- Work with the Director of Clinical Operations to share findings and support process improvement initiatives.
- Maintain accurate and timely documentation through the electronic system.
