Requirements
- Active Occupational Therapist license in the state of employment
- Minimum of one year clinical experience (home health preferred)
- Strong assessment and documentation skills
- Reliable transportation and valid driver’s license
- Ability to work independently in the field
Nice To Haves
- Home health experience preferred
Responsibilities
- Perform initial and ongoing patient evaluations
- Develop and implement individualized plans of care
- Provide therapy focused on activities of daily living (ADLs) and functional independence
- Complete OASIS assessments as required
- Educate patients and caregivers on treatment plans
- Maintain timely and accurate documentation
- Communicate patient status and changes with the care team
