About The Position

FOX Rehabilitation is hiring licensed Occupational Therapists to provide one-on-one care to older adults in Waterloo, Iowa, and surrounding communities. Clinicians work in patients’ private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. The role focuses on improving mobility, independence, wellness, longevity, and quality of life through individualized, evidence-based care.

Occupational Therapists perform evaluations, develop plans of care, provide treatment, document in FOX’s EMR system in accordance with regulatory standards, and educate patients, caregivers, and senior living staff. They collaborate with an interdisciplinary team that includes physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists. Telehealth and other virtual care may also be incorporated when appropriate.

The position offers clinicians autonomy to create and manage their treatment schedules and build lasting relationships with patients. FOX lists full-time, part-time, and PRN/Fox Flex employment pathways. New graduates are eligible for the Emerging Professionals Mentor Program. Compensation varies by experience and employment type; eligible roles may include benefits.

Requirements

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including occasional lifting of 25–50 lbs, frequent lifting of 10–25 lbs, and constant lifting of up to 10 lbs.
• Ability to perform CPR and provide moderate to maximum assistance with patient transfers and bed mobility.
• Ability to reposition patients, stand and walk for extended periods, climb stairs, and perform therapeutic activities.
• Ability to wear PPE when required.

Responsibilities

• Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
