About The Position
FOX Rehabilitation is hiring Occupational Therapists in Plainview, New York, to provide one-on-one care to older adults in private residences and senior living communities. Clinicians help patients improve mobility, independence, and quality of life through FOX’s Geriatric House Calls™ model. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Telehealth and other virtual care may also be incorporated when appropriate. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and a mission to abolish ageism. Clinicians work in a collaborative, interdisciplinary environment and may practice in home and senior living settings. Employment pathways described include full-time, part-time, and PRN/FOX Flex roles; the standard New York base pay range for a full-time role is $70,000–$105,000 annually, with part-time and PRN compensation prorated. New graduates are eligible for FOX’s Emerging Professionals Mentor Program.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including exerting medium physical effort; lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; performing CPR; providing moderate to maximum assistance with patient transfers and bed mobility; repositioning patients; standing and walking for extended periods; climbing stairs; performing therapeutic activities; driving between care locations; and wearing PPE when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and fitness specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Potentially supervise PTA/OTAs, mentor emerging professionals, serve as a clinical instructor, and contribute written or social media content for additional compensation.
