About The Position
FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one care to older adults in Monroe, NY and surrounding communities. Care is delivered in patients’ homes and senior living settings, including independent living, assisted living, and memory care communities, with telehealth or other virtual care available when appropriate. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. The role offers clinician autonomy to create and manage a treatment schedule, build lasting patient relationships, and work within an interdisciplinary team. FOX offers full-time, part-time, and PRN/Fox Flex pathways; compensation and benefit eligibility depend on employment type. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program.
Requirements
- Hold a valid Occupational Therapy license in the state(s) of practice, or be eligible to obtain one.
- Have a degree from an accredited Occupational Therapy program.
- Have basic computer and EMR proficiency.
- Hold current CPR certification.
- Meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living communities.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Conduct therapy sessions that support wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
