We are seeking a Full-Time Occupational Therapist (OT) to join our home care team. In this role, you will provide client-centered, in-home occupational therapy services, helping individuals improve their ability to perform daily activities, enhance independence, and improve overall quality of life—within the comfort and safety of their homes.

Requirements

  • Current, valid Massachusetts Occupational Therapist (OT) license.
  • Valid driver’s license and reliable transportation.
  • Current CPR certification.
  • Minimum 2 year of OT experience (home care experience is required).
  • Strong communication, organization, and time-management skills.
  • Commitment to high-quality, patient-centered care.

Responsibilities

  • Initial assessment/Evaluate clients’ functional abilities and therapy needs in the home setting, in accordance with physician orders.
  • Develop, implement, and regularly update individualized treatment plans focused on ADLs, safety, and independence.
  • Provide hands-on therapy and educate clients and families on adaptive techniques, energy conservation, and home safety.
  • Recommend and train clients on adaptive equipment and assistive devices to support independence at home.
  • Collaborate with interdisciplinary team members including RNs, PTs, caregivers, and physicians to ensure coordinated care.
  • Maintain timely, accurate documentation of evaluations, progress notes, and discharge summaries in compliance with state and agency standards.
  • Deliver compassionate, respectful, and culturally sensitive care to diverse client populations.
