About The Position

YOSVH Rehab is seeking a dedicated Occupational Therapist to provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). The role involves patient evaluations, treatment planning, and collaboration with interdisciplinary teams to enhance patient outcomes.

Shifts vary to provide departmental coverage. The schedule is typically a five-day work week, including weekends as needed. Availability to work on Hawaii State and Federal Holidays is required as operational needs dictate.

Orientation will familiarize the new hire with the rehabilitation department, electronic medical records documentation (Net Health Rehab Optima and Epic), and facility workflows.

Requirements

• Current Basic Life Support (BLS) certification.
• Active State of Hawaii license in good standing, appropriate to the discipline, by the start of the assignment.

Nice To Haves

• A minimum of two years of experience in an SNF/ICF setting is preferred.

Responsibilities

• Provide comprehensive rehabilitation services in an SNF and/or ICF setting.
• Conduct evaluations, reassessments, treatments, and other clinical services appropriate to licensure and scope of practice.
• Develop and implement individualized treatment plans in collaboration with supervising therapists, physicians, and the interdisciplinary team.
• Use evidence-based therapeutic interventions to promote function, independence, communication, mobility, swallowing, and quality of life.
• Educate residents, families, and caregivers about treatment plans and patient progress.
• Maintain accurate, timely, and compliant documentation of patient progress, changes in care plans, and discharges according to facility and regulatory requirements.
• Supervise therapy assistants as required by role and licensure.
• Be available to work on Hawaii State and Federal Holidays as operational needs dictate.
