Occupational Therapist - Home Care (Philadelphia)
About The Position
Helping Hand Nurse, LLC is a home health care agency seeking Physical Therapists for our home health patients. We are actively hiring in multiple locations. Clinicians choose their own schedule, benefit from quick and easy documentation, and have support staff available to assist them. The physical therapist will provide physical therapy services to patients according to a written physician's plan of care, which includes assessing and evaluating therapeutic/rehabilitative/functional status, participating in the development of the total plan of care, and evaluating the home environment. This role involves assessing muscle strength, mobility, gait, ROM, and potential for rehab, directing physical therapy treatment, instructing patients and families/caregivers on the use and care of therapeutic appliances, determining priority needs for physical therapy, and reporting patient reactions to treatment or changes in condition to the physician.
Requirements
- Current unrestricted PT license
- Clean background and criminal record
- CPR certified
- Knowledge of computerized documentation and office software required
Nice To Haves
- Minimum one (1) year of home care experience preferred.
- Three (3) to five (5) years of clinical experience preferred.
- Experience with OASIS (although this can be trained)
Responsibilities
- Visiting patients in their homes and providing skilled home health care clinical visits.
- Providing care for individuals based on a personalized Plan of Care.
