Occupational Therapist - Home Care (Philadelphia)
About The Position
Helping Hand Nurse, LLC is a home health care agency seeking Physical Therapists for our home health patients. We are actively hiring in multiple locations. Clinicians choose their own schedule: -Quick and easy documentation -Make your own schedule -Support staff are always here to help. The physical therapist will provide physical therapy services to patients according to a written physician's plan of care which will, in part, include: Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care, as well as evaluating the home environment. Assessing for muscle strength, mobility, gait, ROM--potential for rehab. Directing physical therapy treatment. Instructing patients and families/caregivers in the use and care of therapeutic appliances. Determining priority needs for physical therapy. Reporting to physician patient's reaction to treatment or changes in condition.
Requirements
- Current unrestricted PT license
- CPR certified
- Knowledge of computerized documentation and office software required
Nice To Haves
- Minimum one (1) year of home care experience preferred.
- Three (3) to five (5) years of clinical experience preferred.
- Clean background and criminal record
- experience with OASIS (although this can be trained)
Responsibilities
- Visiting patients in their homes and providing skilled home health care clinical visits
- Providing care for individuals based on personalized Plan of Care
