Occupational Therapist (OT) Spanish Speaking for Home Health
About The Position
FeldCare Connects, an app-based referral network, is seeking a self-motivated Occupational Therapist (OT) to deliver premier excellence of care. This role involves working with adults of all ages in a home health setting. The OT will be responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on these assessments, the OT will develop treatment plans, perform interventions to improve patient well-being, and monitor progress. This is an independent contractor position serving Dr. Palmdale and surrounding areas.
Requirements
- Ability and enthusiasm in working with all ages
- Occupational Therapist license and registration by the state
- Completion of an accredited Occupational Therapist program

Responsibilities
- Provide services defined by state laws governing Occupational Therapy practice, in accordance with the plan of care, and in coordination with other healthcare team members.
- Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
- Observe, record, and report patient response to treatment and changes in condition.
- Instruct patients, families, caregivers, and the healthcare team on how they can participate in assisting the patient.
