About The Position

The Occupational Therapist (OT) role is the front line of Adaptive Home Health's mission to dramatically improve patient access to home health care. The OT restores function in the home, builds safe routines, reduces avoidable decline, and helps patients regain independence in ADLs/IADLs. Adaptive Home Health builds technology to better support their field clinicians, with a charting product designed to reduce documentation burden and a scheduling product to make coordinating visits easier and protect clinician time. This role offers autonomy, meaningful patient impact, and a support team built to remove administrative friction for clinicians with strong clinical judgment who like owning outcomes.

Requirements

  • Active Texas OT license (in good standing)
  • Strong clinical judgment and comfort working independently in the field
  • Comfort escalating changes in condition to RN/MD and documenting rationale clearly
  • Strong communication and documentation
  • Reliable follow-through on orders, frequencies, and patient goals
  • Current driver’s license and comfort with daily travel across assigned territory
  • Able to carry/transport basic therapy equipment as needed
  • Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice To Haves

  • Prior home health experience
  • Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
  • Strong organization and time management
  • Experience using charting systems / EMRs

Responsibilities

  • Complete in-home OT evaluations and deliver skilled OT visits.
  • Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
  • Recommend and train on adaptive equipment and home modifications as needed.
  • Coordinate with nursing and the broader care team when condition or needs change.
  • Document promptly and thoroughly.
