About The Position

Neuron Health Systems is a community-based provider of acute outpatient rehabilitation and medical and non-medical direct care. We deliver individualized, family-centered support — partnering with families and caregivers and applying an interdisciplinary approach so every patient can reach their fullest potential. The Role We're looking for an Occupational Therapist to help patients build or regain the skills they need for daily life, at home and in their community. This is a hybrid, per-diem role: evaluations, treatment planning, and documentation can largely be done remotely, with in-person visits reserved for hands-on evaluation and treatment sessions. All clinicians are expected to take part in the firm's accreditation activities and surveys as part of maintaining our quality standards.

Requirements

• Active NJ Occupational Therapist license in good standing.
• Comfort working independently in a remote-first workflow, with reliable transportation for in-person visits.
• Strong documentation habits and clear communication with patients, families, and the care team.

Nice To Haves

• Home care, rehabilitation, or I/DD experience preferred.

Responsibilities

• Conduct comprehensive occupational therapy evaluations and functional assessments.
• Develop and implement individualized treatment plans focused on activities of daily living.
• Recommend adaptive equipment and home modifications to support independence.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RNs, PTs, SLPs, and other members of the care team.
• Complete evaluation, progress, and discharge documentation in a timely manner.
• Educate patients and family members on techniques and home exercise programs.
• Participate in case conferences and firm accreditation activities and surveys.
