About The Position

PASCO/SW Home Health is hiring an Occupational Therapist (OT) to provide home health therapy that helps patients regain independence. The role involves creating personalized treatment plans, teaching adaptive skills, and collaborating with a supportive care team. The position offers flexible scheduling and autonomy in patient care with team support. Salary is based on a per-visit model, listed at $40.00–$57.00 per visit.

Requirements

• Licensed Occupational Therapist in the State of Colorado.
• Graduate of an ACOTE-accredited occupational therapy program and eligible for NBCOT certification.
• At least 2 years of clinical experience; home health experience is preferred.
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.

• Home health clinical experience.

Responsibilities

• Develop and implement individualized care plans.
• Provide therapy services according to physician orders.
• Assess patients’ functional status and recommend adaptive equipment.
• Teach new skills or retrain patients in activities of daily living.
• Maintain accurate documentation and participate in case conferences.
